For Baptism, Confirmation, and OCIAThis form is to be used for the potential sponsor/Godparent and should be taken to his/her parish and sign it in the presence of the Pastor or his delegate who will support this with the signature and seal of the Church. Alternatively, the parish may provide its own form, bearing signature and seal, stating that he/she is eligible and prepared to be a Godparent/Sponsor.
